We have just launched the project "Elderly Health and Well-being" within elderly care and offer a developing job in our new close care team. You can use your experiences, express new ideas and thoughts, be part of building knowledge, and work in a team with occupational therapists, physiotherapists, nurses, care managers, and nursing assistants.

Job Description

The close care team is a multiprofessional team that works with discharge support for patients being released from the hospital, intensive rehabilitation aimed at maintaining or increasing the patient's independence, and fall prevention home visits focusing on reducing the risk of falls among elderly individuals.

As a nursing assistant with us, you will be part of the newly established team, which has been operational since the turn of the year 25/26. You will be seen as an important asset to the team and as a bridge builder, and together with your team colleagues, you will shape routines and working methods and be involved in planning and executing tasks assigned to nursing assistants within municipal health and medical care and, to some extent, home care. You will receive extended delegation, and it is essential to build good relationships with the home care staff, coordinators, and planners. Among other things, you will carry the working methods based on a rehabilitative and activating approach together with the other team members.

The close care team is part of the project "Elderly Health and Well-being," where the municipality wants to invest in the elderly and provide them with the best possible conditions to maintain or improve their health and independence, maintain or expand their social context, and be able to stay in their homes as long as possible. The project involves operations within home healthcare, home care, senior centers, and also the contributions of volunteers. The project will run for 5 years and is expected to transition into regular operations after the project period.

Qualifications

You should be an experienced nursing assistant with relevant work experience, at least 5 years. Experience in home care, home healthcare, rehabilitative interventions, teaching, and development work is meritorious.

A B driver's license is required for the position.

You are familiar with IT-based journal systems and can express yourself well in Swedish, both orally and in writing.
